Saaq claim forms

This authorization excludes medical information. Filing your compensation claim Online. Use our online service to file your compensation claim. Using the paper form.


If you prefer to file your claim using the paper form, you must first call us to open a file and be given a claim number. You will need this number to complete the required compensation claim documents. This link will open in a new wndow.


This form is provided to you with the death benefit claim form. If the deceased was receiving Employment Insurance benefits or an employment assistance allowance. You must have the Confirmation of an Employment-Assistance Allowance Lost (PDF, 1KB) form completed.


This service allows you to securely file your claim for compensation. Your application will be filed automatically and you do not need to send a hard copy afterwards. Send your application and cheque by fax or mail to the Service aux accidentés at the Société de l'assurance automobile du Québec. Enclose a cheque marked “VOID. The publications will be delivered to your address.


The deductible is the amount the insured must pay in the event of a claim. Read our blog article about this. If the amount paid by the SAAQ is insufficient, a benefit may be payable by amiabpe insurer. If so, you must first submit your claim to the CSST or the SAAQ. They will send you a Claim for Compensation form.


Ask your doctor to fill out the SAAQ medical report form. Make sure you obtain regular medical follow-ups. If necessary, ask to be referred to a specialist. Find out your rights and obligations. Be diligent about meeting all contestation deadlines.
